Please follow the instructions in this section to expedite your paperwork for your first visit to our office.
In an effort to make your visit as convenient as possible we’ve provided two options for you to choose from for providing your medical history:
- Print forms at home, complete them, and then bring them with you to your first appointment. (Choose either the Child or Adult version or both)

Child Health History Form
NOTE: If you are bringing siblings with the same
insurance and billing party information, only one of the Child Health History forms needs to have that information.
b)
Adult Health History Form
- Arrive 20 minutes prior to your appointment time and complete your forms in the office.
In addition, please print out and read the office policies and HIPPA forms (below), then sign and bring them with you on your first visit to our office.
